As a Federally Qualified Health Center Look-Alike (FQHC-LA), Valley Health Partners (VHP) Community Health Center delivers extraordinary care that is compassionate, accessible and affordable in a family centered and culturally sensitive environment. We aim to be the most accessible health center in the community we serve. Throughout this journey, we have been recognized for the outstanding care we provide to our community.

 

Recognition From Health Resources and Services Administration (HRSA)

Each year, HRSA’s Health Center Program awardees and look-alikes report a core set of information, including data on patient characteristics, services provided, clinical processes, health outcomes and costs and revenues as part of a standardized reporting system known as the UDS.

 

Health Center Quality Leader

In 2023, VHP was again awarded multiple quality badges, including the silver-level health center quality leader – awarded to the top 11%-20% of health centers in the country.

 

Advancing Health Information Technology for Quality

VHP was also awarded a badge for meeting all criteria to optimize health information technology services that advance telehealth, patient engagement, interoperability and collection of social determinants of health data to increase access to care and advance quality of care.

 

Addressing Social Risk Factors

VHP also received a quality badge for addressing social risk factors. This is recognition for the collaborative work and coordination around social risk, from our providers and staff identifying barriers to care in the practices, to our Enabling Services team who work to address those needs directly.
